1. Referral
A physician, hospital, family member, or the patient contacts us and we confirm eligibility.
2. Start of care visit
A registered nurse completes a comprehensive assessment in the home and builds the plan of care.
3. Ongoing visits
Your clinicians visit on a schedule set by your plan of care and report back to your physician.
4. Discharge & follow-up
When goals are met, we review home safety, medications, and next steps with you and your family.

Preparing your home
- Clear walkways and secure loose rugs and cords
